What is the difference between the Declaration of Intent Form and the Legacy Gift Confirmation Form?

The Declaration of Intent Form is a preliminary commitment to leave a legacy gift, while the Legacy Gift Confirmation Form formalizes and finalizes that commitment once the necessary arrangements have been made.

Can I make a legacy commitment to a Jewish organization in our community not listed on these forms?

Yes! Please include the name of your chosen partner organization in the “Other” section on each form. Once completed, return your form to Rachael Evans, Director of Stewardship, at 503-248-9328x100 or rachaele@ojcf.org.
